Handling personal information

Eri Omotesando Clinic (hereinafter referred to as our institute) has established the following policies regarding the processing of personal information.

The terms used in the text are defined by the Law on the Protection of Personal Information (hereinafter referred to as the “Law on Protection of Personal Information”) and related laws.

1. Compliance with laws and related instructions
Our Institute treats personal information in a legal and appropriate manner, in compliance with the Personal Information Protection Law and other relevant regulations issued by the Personal Information Protection Commission and Privacy Policy.
2. What is personal information on our website?
Personal information includes all personally identifiable information. On our website mainly include name, address, age, gender, occupation, phone number, email address, access records, etc. and the information that identifies an individual is considered personal information.

When collecting customers’ personal information on the website, our institute will inform clearly the purpose of use and will collect personal information of customers by appropriate means.     When entrusting the processing of customers’ personal information, our institute will select a partner company to implement appropriate security management measures as a contractor and will provide necessary and appropriate monitoring for with the contractor.
8. Receiving personal information
(1) When a customer or agent requests a notice of the purpose of using personal information, we will respond appropriately and promptly, except in the following cases.
  • A. When the purpose of using personal information to identify customers is clear
  • B. When there is a risk of harm to the lives, bodies, property or other rights and interests of customers or third parties
  • C. When there is a risk of harming our legal rights or interests
  • D. When there is a need to cooperate with a national or local state organization in conducting business in accordance with laws and regulations, or when there is a possibility of obstructing the operation of an enterprise.
(2) We will respond promptly and appropriately to requests for disclosure of personal information to customers or agents, except in the following cases.
  • A. When there is a risk of harm to the lives, bodies, property or other rights and interests of customers or third parties
  • B. When there is a possibility of impeding the proper conduct of our business
  • C. When violating laws and regulations
(3) If there is a request to add or delete personal information from the customer or its agents, we will promptly investigate, from which appropriate action is taken.
(4) When there is a request to stop or delete the use of personal information from customers or their agents, we will respond appropriately after finding out a good reason.
(5) In the case of the above request, please send the document containing the required content along with the identification document to our clinic listed in section 12 below. Personal information provided by the customer will be used for the purpose of meeting customer requirements and is strictly stored. Please note that we will not return attachments as per your request.
9. About the use of cookies
Some of our websites use “cookies” to make them more convenient to use. A cookie is a small text file that is recorded on your computer when you visit the website and thus can identify your computer.

10. Access history
Our institute web site records information such as the page and the date and time that customers visit, such as an access log on the web server of the company we trust. Access log does not collect any information identifying customers. These data are used for analytical purposes to collect the trustee company’s web server statistics and will not be used for any other purpose.
11. Regarding the Processing of personal information
On our website there is a link provided to our related or third party websites to enhance customer convenience. We will not be responsible for the processing of personal information on the linked website. Therefore, we recommend that you read the privacy policy on this site.

13. The revision of the Privacy Policy
